Form D is the City Survey record for plots in towns that were surveyed under the city survey (identified by a PT sheet number and Chalta number rather than a village survey number). It plays the same role as Form I & XIV does for village land.
Register yourself on www.goaonline.gov.in by providing your email ID, mobile number, name, address, and date of birth, followed by OTP verification.
After successful registration , Log in and Navigate to Services Menu -> All Services -> Revenue Department -> Form D.
Select District, Taluka ,Town , PT Sheet Number , Chalta Number from the list.
Click on Preview. Form D Preview will be displayed.
Click Proceed to Payment to apply for a digitally signed copy. The Payment Request Confirmation will be displayed. After confirming the request, the payment details will be displayed.
Click on Pay Now. You’ll be directed to secured payment portal.
After successful payment ,digitally signed copy will be made available for download in your Inbox.
